The idiom Under The Weather indicates that you or someone is not completely well, is sick or in some cultures/countries it could mean that you or someone is drunk or hungover from drinking.
In the case of this image asking someone if they are "Under The Weather?" is asking them if they are not feeling well, possibly sick.
Green was used for the question because we often say or indicate in graphics that someone looks green when they are ill or not well.
Red was used for the final greeting accenting the hope for the person to feel better soon.
